4. Click "Submit".
5. You will then receive an email for your proposed appointment times. Once your appointment request has been reviewed and accepted, you will receive an additional email confirming your appointment.

How To Submit Tax Documents
To submit tax preparation info, scan or take a picture of your documents and e-mail to precise1charlotte@gmail.com.
Be sure to include the following:
1. Contact phone number
2. W-2 or last pay stub of 2017
3. Social Security Card
4. Dependent Date of Birth and supporting documents
5. 1099 if applicable
6. 1095-A Healthcare Form
7. Best time to contact
